This publication includes a general-inquiry contact form. Information submitted through that form is used to handle website questions and corrections.
What the form collects
Required fields include your name, email address, subject, and message. The form sends those details by email to the website administrator’s configured recipient. It does not add a separate message archive to the WordPress database. Email systems and configured delivery services may retain copies.
Do not send medical records, patient identifiers, payment details, or other confidential health information. Contact the appropriate practice using its established patient channels.
Technical processing
The form uses a security token, a hidden field, and a temporary rate limit to reduce automated submissions. The rate-limit record is based on a one-way keyed hash of the submitting connection’s IP address and expires after ten minutes. Hosting and security providers may also process technical request logs.
This package does not install advertising trackers, analytics scripts, or social embeds. WordPress, hosting services, or subsequently installed plugins may process information according to their configuration. Follow external links only if you are comfortable with the destination’s own privacy practices.
Questions about a message
Use Contact for an inquiry about information previously submitted. Provide enough detail to identify the message without including sensitive medical information. The administrator should update this policy if services or data practices change.
